Before you start

This short, easy form asks you to:

• Pick just one category – the best fit for what they do.
• Give a few examples of what they do, who benefits and the difference they make.
• If you are nominating an individual, make sure you have asked their permission to share their contact details such as an email address, with us. We'll use this to contact them about attending a celebration event.
• Write in your own words and give as much detail as you can to help our judges understand why you are nominating them.
